I think first of all, I would like to clarify what STIGMA and DISCRIMINATION means because sometimes the two are confused with each other.
STIGMA - someone sees you in a negative way because of your mental illness
DISCRIMINATION - someone treats you in a negative way because of your mental illness
So STIGMA happens when a person defines someone by their illness rather than WHO they are as an individual. For example, they might be labelled “psychotic” rather than “a person experiencing psychosis”.
Second is to understand what is a mental illness
There are many different conditions that are recognised as mental illnesses. The more common types are:
Anxiety disorders
Mood disorders (eg Bipolar)
Psychotic disorders - hallucinations, delusions , schizophrenia
Eating disorders - Anorexia Nervosa, Bulimia Nervosa, Binge eating
Impulse Control & Addiction disorders (kleptomaniac, pyromania, compulsive gambling)
Personality disorders
OCS
PTSD
Seven Things You Can Do to Reduce Stigma
* Know the facts. Educate yourself about mental illness including substance use disorders.��
* Be aware of your attitudes and behaviour. Examine your own judgmental thinking, reinforced by upbringing and society.��
* Choose your words carefully. The way we speak can affect the attitudes of others.��
* Educate others. Pass on facts and positive attitudes; challenge myths and stereotypes.��
* Focus on the positive. Mental illness, including addictions, are only part of anyone's larger picture.��
* Support people. Treat everyone with dignity and respect; offer support and encouragement.��
* Include everyone. It's against the law to deny jobs or services to anyone with these health issues.

不斷的,聽到來自各地許多病友接到衛生局的「關切」,甚至是地方警察的「關切」、或者是知悉他們罹病病史的鄰居「關切」;假如相關法規所說的「例行關懷」為真,那為甚麼這些個案因為「以前從來沒這樣被打過電話」而感到驚恐? 如果過去並未每日「關懷」的原因是個案病情穩定,那現在一個一個把他/她們當成「有問題」去「關懷」,又是本著哪一個原因?
病情不穩定的,是這個社會吧?
從陸續聽到幾個平常在社區中與住民相處沒有甚麼問題的個案,被「關懷送醫」;甚至有一些病友因為來自莫名的「關懷」加上媒體報導、民眾的不友善,而情緒陷入波動、病情受到影響(個案接到不認識的電話關心病情,會有隱私被揭露的恐懼;況且除非是平常已經建立關係的醫療人員或社工,不然不認識的人一通電話就來討論病情,是很難溝通理解的,更遑論有縣市竟然是由警察來做這個動作!)。
台灣精神醫學會理事長賴德仁教授特別寫了一篇文章,提出了五個重要的看法;我將他的文章貼在文末,誠摯地拜託大家,靜下心來,不要讓這個還在療傷的社會,讓更多脆弱的心靈受傷;拜託大家。

關於劉小妹妹事件的呼籲
台灣精神醫學會理事長 賴德仁教授 2016/4/1
關於劉小妹妹無辜受難的事,我們感同身受。這一陣子,不只劉小妹妹的父母與親友深感難過,相信國人的情緒也跟著受影響。對於此事件,我有以下之看法:
1. 最近新聞媒體持續報導此事,讓我們深感憂心。十年前某藝人自殺死亡後,媒體整天報導,導致那個月自殺率明顯上升,尤其是採取同樣自殺方式的人明顯增多。這不得不讓我們擔心媒體過度的報導,不只讓國人心情持續沉重與難過,也會讓一些高風險的人學習仿效,進而增加社區暴力的風險。
2.在醫師還沒做精神鑑定之前,就有某些單位聲稱加害者是思覺失調症患者,這種未審先判的汙名化令我們憂心。大部份的精神病友在規律治療下,均能保持症狀穩定,他們的犯罪率與暴力行為並不會比一般人來得高。台灣精神醫學會與康復之友聯盟在2014年共同進行精神病人之去汙名化,在獲得衛生福利部支持下,將「精神分裂症」改名為「思覺失調症」,讓精神病友活得更有尊嚴,使得他們在努力復健過程中得到更多的尊重與鼓勵。台灣最美的風景是「人」,所以在此時此刻,請給這些弱勢族群的精神病友與家屬鼓勵與掌聲,不要再汙名化他們了。
3.有關於現行「精神衛生法」強制住院的標準是「嚴重病人傷害他人或自己或有傷害之虞,經專科醫師診斷有全日住院治療之必要者,其保護人應協助嚴重病人,前往精神醫療機構辦理住院。前項嚴重病人拒絕接受全日住院治療者,直轄市、縣(市)主管機關得指定精神醫療機構予以緊急安置,並交由二位以上直轄市、縣(市)主管機關指定之專科醫師進行強制鑑定,而且要送給審查會通過才能強制住院。」這個強制住院標準是達到文明國家重視人權的標準,但是在執行上仍有些許問題,導致強制住院人數急速下降,從原本的每年約三千人降為七百人。所以如何兼顧病人人權與社會安全及家屬照顧負擔,未來需要有關單位與相關專家共同討論。但是我要特別提醒有關單位,不要抱著「寧可錯殺一百,而不錯放一人」的心態,過度緊張,在精神病人並無明顯傷害他人或自己或有傷害之虞,就被強制帶到醫院。
4. 中研院鄭泰安教授的研究發現從1990到2010年,台灣社會常見精神疾病(common mental disorders) 增加一倍,也就是說焦慮、憂鬱、失眠等各種身心問題的人在這二十年期間增加一倍,它與失業率及離婚率呈現顯著正相關,也與自殺率顯著正相關。所以精神疾病的大幅增加是社會問題,也是醫療問題,但不能指望單純用醫療來解決社會問題。另外,近十年基層精神科診所增為三倍,各醫院的精神科門診也有很多患者,所以國人的心理與精神健康已亮紅燈,但是衛生福利部心理及口腔健康司一年有關心理促進及精神醫療之經費僅有五億元。政府漠視精神醫療、醫院評鑑時精神科不被重視、精神科診所及病患人數雖然增加很多但基層健保總額在精神醫療部份卻沒有明顯增加,如此的不良環境,再不改善,未來國內的精神醫療照顧只會更加惡化!整體而言,目前國內的精神醫療的發展障礙有二,誤解太多且資源不足。 汙名化存在排斥精神病友,社區投注資源不足,社區安全網應是友善包容接納的,而非強制隔離肅殺的,如此才能鼓勵就醫,協助家屬與病患獲得完整的醫療與照顧。
5.世界衛生組織一再強調「沒有心理與精神健康,就不可能有真正健康的身體」。建議中央政府成立跨部會的委員會,增加相關預算與人力,以積極的作為來改善國人的心理健康與精神醫療,讓每個孩子都有平安長大的權利,也讓國人能擁有快樂與幸福的人生。我們深信精神醫療是穩定社會人心的重要力量,我們與國人及政府站在一起,撫平大家的恐懼,為共創美好的未來而努力!!
