【高雄氣爆省思──英國工安案例分析】

在工業安全中，最重要的一環，就是建立安全文化──建立每一個階層對安全的共識與承諾。在台灣，上至政府，下至企業，都沒有安全文化。有的是責備文化。責備文化專注於個人所犯的錯誤而不是著重於系統整合、公司組織和管理程序方法。一旦發生可能產生不安全狀況的異常現象或問題，首先考慮到是個人責任承擔。因為怕被罵，所以不敢據實以報。多少重大意外事件的跡象，都因為「責備文化」而被蓋了下來，使得已經被覺察而可以避免的災難一再發生，實在令人扼腕。

為了追查英國Piper Alpha大爆炸工安事件真相，大法官Cullen提出"No Blame to individual"──藉由個人不必承擔全責與壓力，營造信任與安心的氛圍，鼓勵相關人員說出事情真相。經詳細的調查與分析，大法官對於這次工業災害事件，提出106條改善方案，如今還被海上石油天然氣產業奉為圭臬...
