血壓危症 (hypertensive crisis)，包括了 hypertensive urgency 和 emergency 兩
類。
過去會出現惡性高血壓，是因為那時醫學對血壓控制的能力不如現在好，所以預後如同
病人有惡性腫瘤一樣，所以套用了「惡性」這個形容詞。
: 肺水腫有利尿劑使用,
如肺水腫是因高血壓所致，那即符合 hypertensive emergency，應該在最初一小時內
逐漸降低 10~20% 的血壓，減少病人的傷害，多數會使用靜脈給予的藥物，以能較快生
效，也較快能調整劑量
: 當時還有Heparin在使用
有用 heparin 應該是臨床判斷病人也有急性冠心症 (acute coronary syndrome) 吧
: 低鉀KCL補充,但抽血K2.9mEq
病人有使用利尿劑合併低血鉀，應注意是否是利尿劑所致，不管是入院用或者是平時就
有在服用。或者是最初病人即有高血壓合併低血鉀，若是後者還要注意是否有代謝性鹼
中毒，臨床會有要考慮不一樣的疾病。
利尿劑或許對於控制病人的肺水腫是需要的，仍需避免或矯正所帶來的副作用，像是低
血鉀。同時也要留意是否病人有低血鎂，若有病人的血鉀在血鎂未能拉起來前不容易補
回正常值。
: 因血壓高220/102mmHg
: (足背A-line)用NBP量也差不多
: 雙手NBP都143/80mmHg左右
原則上，血壓離主動脈愈遠，SBP 會愈高，DBP 則愈低，而平均動脈壓 (mean BP, MBP)
則不變，但臨床有時較少會管這個，但可以的話，還是以量測近心端的血壓優先。至少
文中提到 ABP 和 NBP 差不多，而且 DBP 也高到 102，所以確實是有血壓高，而且上下
肢有落差。
理想上，上下肢因為肢體大小不一樣，應該配合病人的情況選用不同的壓脈帶，但臨床
很少人這麼作。
: 當時專科護理師有說到
: "可能冠狀動脈阻塞的徵象,
: 所以上下肢血壓落差這麼大"
冠狀動脈阻塞本身應該不會直接連到上下肢血壓不一致，不知道你是在說主動脈狹窄
(coarctation of aorta) 或是周邊動脈疾病 (peripheral artery disease, PAD/PAOD)
。狹窄後面的壓力會「低」於狹窄前，但主動脈狹窄是造成下肢和上肢，或是左右手血
壓不同，而 PAOD 是造成影響到的下肢低於上肢血壓。主動脈剝離 (aortic dissection)
或是其他情況也可能會帶來不同的血壓。
至於若手腳血管不好，全身的血管均有較高的機會也有問題，所以去注意病人是否有冠
狀動脈疾病也是合理的。
: 當中有說到"...就是那個高血壓..5H的症狀"

我也不知道 5H 是指什麼，但查 5H syndrome 會出現一些資料，較有關係的是
syndrome X，5H 包括了 hyperinsulinemia, hyperglycemia, hyperlipoproteinemia,
hypertension, hirsutism，也和多囊性卵巢 (polycystic ovary, PCO) 有關
但臨床上這個應該不是那麼重要（我覺得啦～）
至於 ACLS 裡頭的 5H 也不是一般高血壓就要考量的方向
: 當時病患口服藥Trandate被DC
: 因為她說這個藥根本沒效
: 是因為他作用在周邊血管擴張嗎？
: 查過診斷有惡性高血壓Trandare
: 是可以治療的不懂為何被停掉
Labetalol若病人能吃進去，對於降壓應該滿有效的，甚至可能比 NTG 的降壓效果還強
。當然先不去想 NTG 可以劑量調到很高的情況。
但對於 hypertensive crisis，有時還是以靜脈輸注的藥為首選。
: 如果輸注NTG時有嘔吐情形時,可以
: 馬上關掉嗎？還是需要慢慢調降呢？
: (不知道是否會有反彈現象之類的)
靜脈輸注的藥一般少在高劑量下不調低劑量即關閉，多數會造成病人血液動力學巨大的
變化。當然要調低到多少才關和調低的速度，每個人的看法可能都不一樣。
: 另外我們醫院習慣都是上下調整3ml,
: 這樣是否會有什麼特別用意嗎?
可以注意一下 NTG 的濃度，我們醫院裡是一瓶 250 ml，裡頭有 50 mg 的 NTG，也就是
每 ml 有 0.2 mg。每變動 3 ml 表示每小時輸注的劑量變動了 0.6 mg，換算成每分鐘
即是增減了 10 ug (0.6 x 1000 / 60)。而 NTG 的給予常從 5 ug/min 或 10 ug/min開
始，增減也是 5 或 10 ug/min，即滴數增減 1.5 或 3。至於要調整 1 或 2 或更大的
5、8 也沒人說不行，甚至帶有小數點的，像是 4.3，只是劑量的變動就不是 5 或 10
這樣較簡單的數字了。
: NTG作用主要在減少心臟前.後負荷,擴張冠狀動脈進而降低血壓
NTG 較著重於靜脈壓的改善，因此減少前負荷比較多。當然量大時對於動脈壓力的變化
也會呈現出來。但要注動冠狀動脈的血流來自於舒張期時的流入，所以血壓降太低時反
而心臟會缺血哦！
: CHF病患會使用以減少心臟負荷,
同上，減少前負荷為主，另外是控制肺水腫，及在冠狀動脈疾病下改善心臟血流，有機
會讓心臟的功能進步。
: 如果血壓高劑量往上調整,
: 血壓偏低時會調降甚至先關起來
: 但心跳血壓維持在正常且無呼吸喘時,
: 就暫時不使用了嗎？
如果血壓正常或呼吸不喘是因為有 NTG 的作用，那停用不就血壓又不正常或呼吸喘起
來了呢？
當然也不是停用病人就又回到原來不好的情況這麼簡單，心血管有進步就可以考慮劑量
減低或停用。
另外怎樣的血壓才是低並沒有絕對的數字，有人 SBP 120 mmHg 就太低了，有人 95
mmHg 仍夠高，數字需要臨床判斷、需要考量很多層面或參考別的數據後才有意義。
: 病患有常規利尿劑使用,尿量也解的很好
: 因為覺得是CHF所以使用NTG,
: 是不是有一直continue使用的必要?
NTG 能改善 CHF 的某些症狀，如常見的肺水腫的喘，但不一定能改善心臟的功能，而
尿量要能出來，一部分是心功能要夠，有足夠的血液灌流，一部分是利尿劑的作用。不
一定是 NTG 的作用哦！所以停不停 NTG 不是說尿量好就單純作為依據，臨床也較少用
這個作為 NTG 變動的依據。
利尿劑的部分並非病人一定得一直服藥，得看心臟功能和瓣膜是否改善，是否有用藥物
來改變病人的血流動力學，以及腎功能的情況。病人用了水若脫太多造成組織灌流不足
，或是不用水積太多因而水腫或呼吸喘，均是不理想的狀況。
病人終究得朝向停掉靜脈藥物的方向去，不然難道要病人帶著 NTG 回家？所以目標仍
是要停，但不少會轉成口服的類似藥物，像是 isosorbide dinitrite (Isordil)、
mononitrite (Imdur) 或 nicorandil 等。但也要知道的是，並非病人有 CAD 就非得
吃這類藥，而且它們並不會改善患者的存活率。
