老師好幾年前寫過一篇文章: 抗精神病藥物與錐體外症候群,因為這篇文章,在網路上常有人問我相關的問題,最近有一位電視上算知名的人物,傳訊息問這類的問題,讓我又想起來這篇文章。
雖然有點久,但是今天還是拿出來和同學分享一下。我會拆成幾篇,每天跟大家分享一點。
前 言
在治療精神分裂症(schizophrenia)時,使用傳統的抗精神病藥物容易產生錐體外徑症狀(extrapyramidal symptoms),這種副作用會使病人有肢體上運動的困難。1989 年之後在美國上市的clozapine 可以有效減少錐體外徑症狀。這類新的治療精神分裂症的藥物,我們就稱它們為非典型抗精神病藥物(atypical antipsychotics)。之後發現新一代的抗精神病藥物與傳統抗精神病藥物比較,其藥物副作用,如錐體外徑症狀明顯改善,且心臟血管、癲癇等問題也少很多。
錐體外徑症狀之定義
錐體外徑症狀是指控制運動之系統受到影響,其神經系統稱為錐體路徑(pyramidal tract)。錐體路徑包括:基底核(basal ganglia)、視丘下核(subthalamic nucleus)、黑質體(substantia nigra)、紅核(red nucleus)和腦幹網狀系統(brain stem reticular formation)。其中傳統抗精神病藥物阻斷了黑質紋狀體多巴胺徑路(nigrostriatal dopamine pathway)之多巴胺D2 受體(dopamine D2 receptors),而容易產生運動方面的副作用。
