婦科疾病再一發 --> #子宮內膜異位

今日流血🩸流到很厭世，決定來跟大家講講這個討厭的疾病 <子宮內膜異位症>

簡單來說，就是子宮內膜組織長在不是子宮內膜的地方~~(繞口令?)

✏️長到卵巢的叫做「巧克力囊腫 (endometrioma/chocolate cyst」；長在子宮肌肉層裡的叫做「子宮腺肌症/肌腺症 (adenomyosis)」。其實他們都是一樣的東西~

🔎那這些疾病常引起哪些症狀?
不外乎經痛、經血量過大、慢性骨盆腔疼痛...

因此當許多因為經痛，或是月經量很大/有血塊的女生們來就診時，會安排超音波來排除結構上的問題。

#如何用超音波診斷子宮內膜異位?

🔎子宮肌腺症

正常子宮約拳頭大小。如果子宮變大，形狀變圓球狀，或是子宮前後壁不均勻的變厚，都像是子宮肌腺症的特徵。

有時候一大團的肌腺組織也會看起來像是一顆圓球狀，但是邊界又不像是一般子宮肌瘤這麼明顯時，會稱之為子宮肌腺瘤(adenomyoma)。

🔎巧克力囊腫🍫

卵巢部分出現囊腫，中間質地均勻，顏色偏灰階。

#還有其他種子宮內膜異位的形式嗎?

當然有!

大家會比較印象深刻的，可能就是某女性發現她每個月固定時間都會咳血，後來檢查發現是子宮內膜異位跑到胸腔🫁。
-->這當然是比較極端且少見的例子

✏️子宮內膜異位侵犯的地方無奇不有，會跑到鼻黏膜👃🏼、胸腔🫁、骨盆腔、腹腔的器官上、腸子上...

只是其他器官的內膜異位，就無法用超音波來做診斷了...

✔️如果有腸道疼痛，或是肛門直腸疼痛的人，可以藉由大腸鏡輔助來看是否有腸道上的子宮內膜異位。

✔️至於骨盆腔的內膜異位，通常都是只能靠肉眼看到(也就是開刀)才能確診。

✔️因此，如果是懷疑深部浸潤型子宮內膜異位的患者，如果沒有特別造成腹腔沾黏、輸尿管水腫等症狀，超音波也無法看到病灶。

#超音波以外還有哪些檢查可以做?

最常合併會做的檢查應該就是抽血 #CA125，「子宮內膜異位」也會造成CA-125升高!!!

這裡就要提一下，現在健康檢查很普及，門診也很常碰到病人因為去做健康檢查，抽了一堆腫瘤指數，然後意外發現CA-125升高而轉診過來的。

但是，會造成CA-125上升的原因很多，不是升高就是卵巢癌!!!

✏️常見婦科的原因:
月經經期、子宮內膜異位症、卵巢腫瘤、骨盆腔發炎...

因此碰到這類型的病患，我們通常會安排婦科超音波，來排除子宮以及卵巢的病灶。如果沒有以上這些原因，子宮卵巢也看起來都沒事，就不要太擔心是婦科的問題!

至於要不要治療? 要不要開刀? 要不要吃藥? 能不能懷孕?
每個人的狀況都不一樣，而且治療方式 太~~多~~了~~
因此建議直接和幫你檢查的婦產科醫師討論喔!

其實子宮內膜異位會增加成孕的難度，就算成功懷孕，孕期腹壓增加亦有機會令朱古力瘤破裂。所以如果發現有朱古力瘤，還是要諮詢醫生跟進治療較穩妥。同時也要注意飲食，不宜生冷、進補。

Ms. Ho: “A friend of mine once had a chocolate cyst, but it disappeared after she gave birth to a child. Is it possible?”

CheckCheckCin: Endometriosis (or chocolate cyst) occurs when the endometrium grows out of its designated areas and turns into a blood tumor. The endometrial lining of the uterus is made of menstrual blood. During pregnancy, ovulation would stop. Hence, the endometrium would stop developing.

Since menstruation will not take place during the pregnancy period, the symptoms caused by chocolate cyst will be less obvious. In some cases, the size of the chocolate cyst will even reduce in size. The tumor, however, can still be found in the womb and might recur after delivering the baby.

Women with endometriosis will have a more difficult time to conceive. Even if the attempt is successful, the pressure within the womb during the pregnancy might cause the chocolate cyst to burst. It is best to consult doctors and seek proper treatment if a chocolate cyst is detected. At the same time, watch your diet, avoid eating raw and cold food, and avoid nourishing the body inappropriately.
