新型口服抗血劑 NOAC
NOAC的正確名稱是什麼？
NOAC：新型口服抗凝劑Novel Oral Anti-Coagulant，
NOAC：有些是寫Non–vitamin K oral anticoagulants (NOAC)，非維生素K口服抗凝劑。這個是美國心臟學院(ACC)在2019的指引使用的名稱。
另一種寫法是DOAC，direct-acting oral anticoagulants (DOAC)，直接作用於凝血因子之口服抗凝劑。
Anyway, 以上都是一家人，通常以NOAC稱之，也就是目前使用的四種藥：
Dabigatran、Rivaroxaban、Apixaban及Edoxaban。
非瓣膜性vs瓣膜性
NOAC藥物剛上市時，是針對對非瓣膜性心房纖維顫動(non-valvular AF, NVAF)。建議使用口服抗凝血劑來預防血栓的研究論文都特別註明，為非瓣膜性心房纖維顫動(non-valvular AF, NVAF)而定。2016年歐洲心臟學會之AF治療指引(ESC guideline for the management of AF patients)中，即不再使用 “non-valvular AF”這個名詞。因為20%患者合併有各式瓣膜性心臟病或是曾做過瓣膜成型術或換過生物瓣膜，在這些患者的次分析顯示NOAC仍保有相當的效果與安全性，所以反而是令人混淆的名稱。而後2018 EHRA、 2019 AHA/ACC更新的AF治療指引，都不再使用non-valvular AF。
目前valvular AF定義：
瓣膜性心房纖維顫動(valvular AF)專指中度至重度二尖瓣狹窄(moderate to severe mitral stenosis，很可能需進行瓣膜手術)及機械瓣膜(mechanical valve) 置換者。
這些患者建議需長期使用warfarin 並保持適當的INR以有效預防血管栓塞。而對其中機械瓣膜置換患者(不含生物瓣膜或瓣膜 成形術)特別強調建議使用 #VKA (warfarin) 而非NOAC才能確保有效預防腦梗塞的發生。
2019 ACC指引也特別指出，以前稱為「抗血栓劑」(antithrombotic)，現在改為「抗凝劑」(anticoagulant).
