Relevant prevention measures for the upcoming semester of the Office of Academic Affairs

各位好:
因應嚴重特殊傳染性肺炎疫情發展,除校方發布各項防疫措施外,有關教學及其相關申辦業務,請各單位依據以下措施配合辦理:
一、110-1學期開學後前三週全面採遠距教學,授課教師應將上課網址及課程相關訊息公告於課程大綱 (有關NTU COOL課程網頁如何設為公開,詳見 https://www.dlc.ntu.edu.tw/2021/09/09/visibility/ )。實驗、實作或人數較少的研究所課程若需採實體授課,須依以下規範辦理:
(一) 落實課堂點名並禁止在教室內飲食。
(二) 上課教室應保持通風良好及定時清消。
(三) 全程配戴口罩,並維持安全社交距離 (1.5米或2.25平方米)。
(四)於110年9月24日前提出申請送教務處核定:申請表詳見課務組公告 https://www.space.ntu.edu.tw/navigate/s/CB679CFFA6134C83B4964E40A30A460AQQY
二、開學後第四週起上課方式,修課人數80人(含)以上課程採遠距教學,80人以下以實體課程為原則,特殊情形請於110年10月12日前提出申請送教務處核定:申請表詳見課務組公告 https://www.space.ntu.edu.tw/navigate/s/CB679CFFA6134C83B4964E40A30A460AQQY
(一) 採實體授課者,仍須依前述防疫規範辦理。
(二)本校鼓勵教師以數位和實體並行的混成方式進行教學準備,詳細資訊請見數習中心網站: https://www.dlc.ntu.edu.tw/2021/09/08/blended-teaching/
三、本學期加退選、停修彈性做法如下:
(一) 解除擋修、超修及減修申請書: 老師及系主任可親簽或用郵件代替簽名,請同學將申請書及相關同意證明以email寄至所屬教務單位承辦人。
(二) 開學後第一、二週加退選:
1.第1、3類方式加選:照舊(上網登記分發)。
2.第2類方式加選(授權碼): 建議老師可用信件傳遞授權碼予學生加選,並記錄學號留存,以利日後查核。
(三) 開學後第三週人工加選及停修:老師及系主任可親簽或用郵件代替簽名,請同學將申請書及相關同意證明,合併成1個pdf檔上傳至教務處。上傳網址將再另行公告。
(四) 因本學期前3週皆採遠距授課,同學在加退選期間若有加簽或聽課需求,請主動聯繫授課老師。

Dear NTU community members,
Given the current pandemic situation, the University is asking all academic and administrative units to comply with the following measures regarding teaching and learning for the upcoming semester despite the epidemic prevention measures released by the school:
A. All courses shall adopt distance learning in the first three weeks since 110-1 semester. Teachers shall announce course URL and relevant information on course description.(If you want to set the visibility of your course page on NTU COOL to be public, please see this document: https://www.dlc.ntu.edu.tw/2021/09/09/visibility/ ) If experiments, hands-on course,graduate courses with fewer people,wish to give physical course, please follow the measures listed below:
1. Teachers shall do the roll call, while eating and drinking in the classroom is forbidden.
2. Classrooms shall be cleansed and disinfected regularly, while kept ventilated.
3. Face masks must be worn throughout, while social distancing shall be maintained (e.g., 1.5 meters or 2.25 square meters).
4. Please submit an application form ( https://www.space.ntu.edu.tw/navigate/s/CB679CFFA6134C83B4964E40A30A460AQQY ) to the Office of Academic Affairs before September 24th.
B. In principle, 80 or above people selecting a course shall adopt distance learning from the fourth week of 110-1 semester. Courses under 80 people will be in-person classes. In special cases, please fill out this application form ( https://www.space.ntu.edu.tw/navigate/s/CB679CFFA6134C83B4964E40A30A460AQQY ) before October 12th.
1. In-person class shall comply with the regulations listed above.
2. Teachers are encouraged to adopt a blended teaching approach -a combination of online learning and in-person class - to prepare the courses. For more information, please see the website of Digital Learning Center: https://www.dlc.ntu.edu.tw/2021/09/08/blended-teaching/
C. The flexible measures of this semester for adding, dropping, and withdrawing classes are as follows.
1. The “Unblock Course Request Form”, “Course Overload Request Form”, and “Course Underload Request Form”:
The lecturer and the director may sign these request forms in person or reply by email to these request forms to indicate their approval of the request. Please email the request form and the proof for approval to the contact person in the Office of Academic Affairs.
2. Adding and dropping classes in the first and second week of this semester:
(1) . Adding classes in the first and the third category:
The process is as usual. Students may register for classes online.
(2). The second category (adding classes with a permission number):
It is recommended that the lecturer email a permission number to students and record the student number for future verification.
3. Adding or withdrawing classes in the third week of this semester:
The lecturer and the director may sign the application form in person or reply by email to the application form to indicate their approval of the request. Please combine the application form and the proof for approval into a single PDF and upload it to the Office of Academic Affairs. The website address for uploading the file will be announced separately.
4. Because online teaching is conducted during the first three weeks of this semester, if students want to add or attend the classes during the period, they need to contact the lecturer.

因應疫情升溫,有關課程、學位考試、轉系申請等事宜,說明如下:
一、課程:
(一)自5月17日起至6月11日全校課程皆採遠距教學。
※若因課程性質確實無法採遠距教學方式(例如:實驗課、實作課、體育課、服務學習課程),採事後補課,原則上於6月30日前完成(視疫情狀況調整)。
(二)課程考試:5月17日至5月28日兩週暫停實體考試(減少學生群聚,兩週後方式視疫情評估後宣佈)。
(三)實施遠距教學,仍需掌握學生到課狀況、學習情形,並檢核教學成效。(教育部5月15日通報)
二、研究生學位論文口試:自5月17日起至6月11日止,以採線上(視訊)方式舉行為原則(無需事先申請),若併以實體會議進行口試,會場社交距離須至少 1.5公尺,並全程佩戴口罩。
三、轉系申請:請將徵得原就讀學系/院學士班主任、導師(研究生為原就讀系/所/學位學程指導教授及主管)同意之email併同申請表寄給註冊組承辦同仁辦理,相關細節將於5月18日公告於註冊組網頁。

Dear faculties and students,
In response to the elevation of COVID-19 threat level, the followings are the matters regarding the courses, examinations, transfer applications, and others.
1. Courses: All courses in the school adopt the remote teaching method from May 17th to June 11th. If it is impossible to use the remote teaching method due to the nature of the course (for example, laboratory courses, practical courses, physical education courses, service-learning courses), make-up courses must be offered after June 11th. In principle, all make-up courses must be completed before June 30th (subjected to change depending on the pandemic situation). Note that during the period of remote teaching, teachers must pay attention to student’s learning outcomes and check teaching effectiveness (according to the notification from the Ministry of Education on May 15th).
2. Course exams: In-person classroom exams will be suspended for two weeks from May 17th to May 28th in order to minimize the student’s gathering indoor (whether this temporary suspension is continued after May 28th will be announced depending on the pandemic situation).
3. The oral exam of MS and Ph.D. thesis defense: It is recommended that all thesis defenses are held online from May 17th to June 11th. No prior permission application is required. If the thesis defense must be conducted in-person in a room, the social distancing rule of 1.5 meters must be applied, and masks must be worn at all times.
4. Application for transfer: No need to obtain the signatures from the academic mentor and the department head during the period from May 17th to June 11th. Email all relevant documents (including the email records demonstrating the approvals from the academic mentor and the department head) to the Office of Registration for processing. The details will be announced on the website of the Office of Registration by May 18th.

因應疫情升溫,本校將自5月17日(星期一)起加強防疫措施,包括 #全校課程改採遠距教學,#校外人車除因公務不得入校。
一、 全面遠距教學:
(一)若因課程性質確實無法採遠距教學方式(如實驗課、實作課、體育課),須事後補課,原則上於6月30日前完成。
(二)授課教師得依課程需求,進行實體或線上評量(含考試),實體評量須依防疫第二級規定(量溫消毒、全程戴口罩、梅花座、教室保持通風、落實課程點名等措施)進行。

二、 校外人士、車輛不得入校:
(一) 管制方式:
1. 教職員工生出入校須出示教職員工生證件。已有本校車輛通行證者得直接通行。
2. 因公務入校者必須出示受訪單位提供的證明文件(如開會通知、邀請函、電郵等,紙本或電子檔皆可),無證明文件者由受訪單位派員至管制口確認身分後方可入校。
(二) #外送員得依本校規範入校送餐或送貨:
1. 外送員於校門口配合量測體溫(額溫需低於37.5℃)及配戴口罩。
2. 請各館舍規劃「外送餐點及物品取餐處」,現場請備妥桌、椅及消毒酒精等防疫物資。
3. 外送員嚴禁進入系館,外送餐點及物品採不接觸取餐(貨)。
三、學生社團活動
(一)學生社團活動立即停辦或改為線上辦理。
(二)課外組管理的場地如社團辦公室、蒙民偉樓以及風三國際學生活動中心等,5月17日中午12點起閉館,如須取回社辦內之私人或需用物品,請於當天11點前取回。
(三)服務學習課程自5月17日起改採遠距教學。

Dear Faculty, Students and Staff:
In response to continuous levelling in the epidemic severity, NTHU has raised our epidemic prevention measures. Starting from May 17th (Monday), all courses will be conducted remotely. Except for official needs, all personnel and vehicle outside of NTHU are prohibited from entering the campus.
Additional explanation:
I. Full remote courses:
A. If remote classes are not possible due to the nature of the course, (e.g., laboratories, hands-on classes, and physical education class), the classes will be made up later, but in principle before June 30th .
B. Instructors could decide to have physical or online assessments (including examinations) at their own discretion. The physical assessments must be in accordance with the second level of epidemic prevention regulations (temperature measuring, disinfection, full-time mask wearing, social distancing, ventilation in the classroom, taking attendance, etc.)

II. Non-NTHU personnel and vehicle control:
A. Campus entrance control:
1. All faculty, student, and staff are required to show their NTHU identification card. Those who have the NTHU vehicle pass can enter directly.
2. Visitors on official business must provide document issued by the inviting unit (such as notice of meeting, invitation letter, email either in electronic form or hard copy). Visitor without documentation must be verified and accompanied by personnel from the inviting NTHU unit.
B. Delivery personnel:
1. Delivery personnel must have body temperature below 37.5℃ measured at NTHU gates and wear a medical mask to be allowed to enter.
2. All buildings need to organize a deliver zone, with properly arranged desk, chair, and alcohol.
3. Delivery personnel are strictly prohibited to enter the buildings and cannot have physical contact with NTHU personnel.
