今早為Asian Medical Students Association Hong Kong (AMSAHK)的新一屆執行委員會就職典禮作致詞分享嘉賓,題目為「疫情中的健康不公平」。
感謝他們的熱情款待以及為整段致詞拍了影片。以下我附上致詞的英文原稿:
It's been my honor to be invited to give the closing remarks for the Inauguration Ceremony for the incoming executive committee of the Asian Medical Students' Association Hong Kong (AMSAHK) this morning. A video has been taken for the remarks I made regarding health inequalities during the COVID-19 pandemic (big thanks to the student who withstood the soreness of her arm for holding the camera up for 15 minutes straight), and here's the transcript of the main body of the speech that goes with this video:
//The coronavirus disease 2019 (COVID-19) pandemic, caused by the SARS-CoV-2 virus, continues to be rampant around the world since early 2020, resulting in more than 55 million cases and 1.3 million deaths worldwide as of today. (So no! It’s not a hoax for those conspiracy theorists out there!) A higher rate of incidence and deaths, as well as worse health-related quality of life have been widely observed in the socially disadvantaged groups, including people of lower socioeconomic position, older persons, migrants, ethnic minority and communities of color, etc. While epidemiologists and scientists around the world are dedicated in gathering scientific evidence on the specific causes and determinants of the health inequalities observed in different countries and regions, we can apply the Social Determinants of Health Conceptual Framework developed by the World Health Organization team led by the eminent Prof Sir Michael Marmot, world’s leading social epidemiologist, to understand and delineate these social determinants of health inequalities related to the COVID-19 pandemic.
According to this framework, social determinants of health can be largely categorized into two types – 1) the lower stream, intermediary determinants, and 2) the upper stream, structural and macro-environmental determinants. For the COVID-19 pandemic, we realized that the lower stream factors may include material circumstances, such as people’s living and working conditions. For instance, the nature of the occupations of these people of lower socioeconomic position tends to require them to travel outside to work, i.e., they cannot work from home, which is a luxury for people who can afford to do it. This lack of choice in the location of occupation may expose them to greater risk of infection through more transportation and interactions with strangers. We have also seen infection clusters among crowded places like elderly homes, public housing estates, and boarding houses for foreign domestic helpers. Moreover, these socially disadvantaged people tend to have lower financial and social capital – it can be observed that they were more likely to be deprived of personal protective equipment like face masks and hand sanitizers, especially during the earlier days of the pandemic. On the other hand, the upper stream, structural determinants of health may include policies related to public health, education, macroeconomics, social protection and welfare, as well as our governance… and last, but not least, our culture and values. If the socioeconomic and political contexts are not favorable to the socially disadvantaged, their health and well-being will be disproportionately affected by the pandemic. Therefore, if we, as a society, espouse to address and reduce the problem of health inequalities, social determinants of health cannot be overlooked in devising and designing any public health-related strategies, measures and policies.
Although a higher rate of incidence and deaths have been widely observed in the socially disadvantaged groups, especially in countries with severe COVID-19 outbreaks, this phenomenon seems to be less discussed and less covered by media in Hong Kong, where the disease incidence is relatively low when compared with other countries around the world. Before the resurgence of local cases in early July, local spread of COVID-19 was sporadic and most cases were imported. In the earlier days of the pandemic, most cases were primarily imported by travelers and return-students studying overseas, leading to a minor surge between mid-March and mid-April of 874 new cases. Most of these cases during Spring were people who could afford to travel and study abroad, and thus tended to be more well-off. Therefore, some would say the expected social gradient in health impact did not seem to exist in Hong Kong, but may I remind you that, it is only the case when we focus on COVID-19-specific incidence and mortality alone. But can we really deduce from this that COVID-19-related health inequality does not exist in Hong Kong? According to the Social Determinants of Health Framework mentioned earlier, the obvious answer is “No, of course not.” And here’s why…
In addition to the direct disease burden, the COVID-19 outbreak and its associated containment measures (such as economic lockdown, mandatory social distancing, and change of work arrangements) could have unequal wider socioeconomic impacts on the general population, especially in regions with pervasive existing social inequalities. Given the limited resources and capacity of the socioeconomically disadvantaged to respond to emergency and adverse events, their general health and well-being are likely to be unduly and inordinately affected by the abrupt changes in their daily economic and social conditions, like job loss and insecurity, brought about by the COVID-19 outbreak and the corresponding containment and mitigation measures of which the main purpose was supposedly disease prevention and health protection at the first place. As such, focusing only on COVID-19 incidence or mortality as the outcomes of concern to address health inequalities may leave out important aspects of life that contributes significantly to people’s health. Recently, my research team and I collaborated with Sir Michael Marmot in a Hong Kong study, and found that the poor people in Hong Kong fared worse in every aspects of life than their richer counterparts in terms of economic activity, personal protective equipment, personal hygiene practice, as well as well-being and health after the COVID-19 outbreak. We also found that part of the observed health inequality can be attributed to the pandemic and its related containment measures via people’s concerns over their own and their families’ livelihood and economic activity. In other words, health inequalities were contributed by the pandemic even in a city where incidence is relatively low through other social determinants of health that directly concerned the livelihood and economic activity of the people. So in this study, we confirmed that focusing only on the incident and death cases as the outcomes of concern to address health inequalities is like a story half-told, and would severely truncate and distort the reality.
Truth be told, health inequality does not only appear after the pandemic outbreak of COVID-19, it is a pre-existing condition in countries and regions around the world, including Hong Kong. My research over the years have consistently shown that people in lower socioeconomic position tend to have worse physical and mental health status. Nevertheless, precisely because health inequality is nothing new, there are always voices in our society trying to dismiss the problem, arguing that it is only natural to have wealth inequality in any capitalistic society. However, in reckoning with health inequalities, we need to go beyond just figuring out the disparities or differences in health status between the poor and the rich, and we need to raise an ethically relevant question: are these inequalities, disparities and differences remediable? Can they be fixed? Can we do something about them? If they are remediable, and we can do something about them but we haven’t, then we’d say these inequalities are ultimately unjust and unfair. In other words, a society that prides itself in pursuing justice must, and I say must, strive to address and reduce these unfair health inequalities. Borrowing the words from famed sociologist Judith Butler, “the virus alone does not discriminate,” but “social and economic inequality will make sure that it does.” With COVID-19, we learn that it is not only the individuals who are sick, but our society. And it’s time we do something about it.

[Kinh nghiệm học TOEFL]_Please help to share and tag your friends. Vì có rất nhiều bạn học TOEFL để apply đi Mỹ và nhắn chị post thêm kinh nghiệm vì trước đây chị hay post IELTS hơn, hôm nay chị post cho cả nhà kinh nghiệm học TOEFL nhé :). Mọi người share và tag bạn bè nhé.
Chào mọi người,
Mình thi TOEFLiBT hôm 14/2/2009. Điểm của mình ko được cao lắm (Total: 108; Reading: 29, Listening: 27, Speaking: 24, Writing: 28). Tuy nhiên, mình có một thời gian khá dài ôn thi và đi dạy TOEFLiBT. Nên mình mong rằng những kinh nghiệm tổng hợp về ôn thi TOEFLiBT dưới đây có thể có ích cho những bạn nào muốn đạt điểm trên 100.
1) Lịch ôn: Bạn nên đặt lịch cho việc học TOEFL, trong đó có phân chia các giờ trong mỗi ngày. Đặt mục tiêu mỗi ngày dành mấy tiếng học TOEFL, vào giờ nào học kĩ năng nào thì thích hợp. Bạn cũng nên dành ra 1 khoảng thời gian dài ko bị distract (khoảng 4 - 6 tiếng) để làm test thử. Khá căng thẳng đấy. Nhưng đó là cách tốt nhất để ôn. Practice với nhiều đề thi, rút kinh nghiệm riêng sau từng đề thi
2) Lập nhóm: Nếu có thời gian bạn có thể xin tham gia làm thành viên hoặc tự lập một nhóm tự học riêng. Học nhóm ko chỉ giúp bạn có thêm nhiều nguồn thông tin bổ ích và motivation để học mà quan trọng hơn là bạn sẽ có thêm nhiều người bạn rất tốt, thậm chí trở thành thân thiết tri kỷ (như nhóm TOEFL10 của mình). Khi luyện nói, nếu ko gặp mặt được trực tiếp, thì các bạn có thể dùng Skype hoặc Paltalk.
3) Sách và CD-ROM:
Bạn nên luyện các chương trình sau (mình có mà ko đủ time để luyện hết --> hơi phí)
- Longman (8 CDs, 1 CD-Rom): sách này cho trình độ A – cơ bản. Listening và reading khá dễ. Tuy nhiên, phần writing và speaking ko giống đề thi lắm nên có thể bỏ qua. Nếu background của bạn khá rồi thì ko cần học qua sách này
- Delta Key (10 CDs): Các skills được trình bày khá kĩ, đặc biệt là có khá nhiều bài tập để practice. Trình độ cao hơn Longman.
- Princeton Review (1 CD): Các tips trong sách này khá hay. Phần Speaking, Writing có các templates để bạn áp dụng cho bất kì topic nào.
- Barron's (10 CDs + 1 CD-ROM): Quyển sách ko thể thiếu trong quá trình luyện thi. Level ngang ngửa hoặc hơi khó hơn so với đề thi thật (nhất là phần listening). Cả 4 skills đều rất tốt, và 4 academic skills là taking notes, paraphrasing, summarizing and synthesizing cực kỳ hữu ích. Ngoài ra còn có 7 full tests để bạn practice. Bạn của mình chỉ practice 7 full tests trong quyển này mà Reading và Listening đều đạt 28, 29. Tốt nhất là nên làm 7 full tests trong cả CD-ROM nữa để quen với cách thi trên máy tính.
- Kaplan (CD-ROM): Mình có sách này nhưng thấy ko hay lắm. Còn 4 full tests trong CD-ROM thì rất tuyệt. Nhiều người ở TestMagic đạt điểm tuyệt đối (120) hoặc gần tuyệt đối (118+) đều recommend 4 full tests này.
- ETS Official Guide (1 CD): 1 sách TOEFL ko thể thiếu. Phần reading và listening thì hơi dễ, nhưng phần speaking và writing thì khá chi tiết và hay.
4) Educated guess: nghĩa là ko phải tick bừa kiểu lottery. Phải cân nhắc thật kĩ, loại trừ càng nhiều đáp án sai càng tốt. Sau đó, còn lại khoảng 2-3 đáp án thì cân nhắc bằng cách tìm supporting details trong bài. Câu nào có supporting details trong bài thì là đáp án đúng
5) Làm đề: Bây giờ cách tốt nhất là luyện đề, và làm đề trên máy. Làm đề nào xong thì dành thời gian để xem đáp án của mình, đọc explanation của họ, xem tại sao mình sai, rút kinh nghiệm để lần sau ko bị sai như thế (có thể làm 1 cuốn notebook ghi lại lỗi sai hay gặp và bài học rút ra)
6) Tinh thần: Mới bắt đầu học thì dễ nản lắm đấy, nhất là khi background chưa tốt. Nhưng cố lên. Practice nhiều thì sẽ lên. Khi đó càng có hứng thú học hơn. Chứ ngay từ đầu đã giỏi rồi thì cần ôn làm gì.
7) Học từ mới: học bằng flashcard hiệu quả lắm. Hồi mình ôn GRE 2 tháng, có ngày học đến 200 từ mới. Thật ra như vậy ko tốt lắm cho long term memory, nhưng vì thời gian ôn gấp quá, phải học nhồi nhét cho kịp thi. Học để thi, thi xong quên luôn. Hic.
Còn học TOEFL thì ko cần quá nhiều từ cao siêu. Mình học theo 2 lists flashcard. Flashcard TOEFL success ko biết ai làm mà khá sơ sài. Còn Flashcard 400MustHaveTOEFLWords là do nhóm TOEFL10 của mình chia nhau ra làm rất cẩn thận, lấy nghĩa tiếng Anh trong quyển sách 400MustHaveWordsfortheTOEFL của McGrawHill, ngoài ra bọn mình còn thêm nghĩa tiếng Việt và examples nữa. Các bạn in 2 mặt rồi cắt ra mà học.
Ngoài ra thấy từ mới nào thì tự viết ra flashcards. Lúc nào cũng kè kè flashcard, vừa đi đường vừa đếm như đếm tiền. Nói chung là ko nên mất thời gian để học từ. Nên tận dụng tối đa time rảnh để học flashcard. Nên học thêm những từ về academic topics.
Nếu ai có nhiều time để ôn thì có thể học từ bằng cách register với các trang như Word smart, Your Dictionary ... Mỗi ngày họ sẽ gửi vào mailbox của bạn 1 từ. Học 1 thời gian dài thì cũng biết thêm được nhiều từ phết, mà lại nhớ được lâu.
8) Học chiến thuật làm bài (tips/strategies) rồi practice để thành thạo. Đừng hy vọng là sẽ vào topic dễ kẻo thất vọng. Trái lại, trong lúc luyện thì nên đọc text với các topics khó và lạ, để quen dần, ko còn cảm giác bỡ ngỡ và nản khi thấy 1 bài text khó nữa. Cái này thì nên dùng Barron's, vừa khoai vừa lạ hoắc trong phần nghe và đọc
9) Reading: Mỗi người có một cách khác nhau để làm bài đọc. Có người đọc câu hỏi trước, có người đọc toàn bộ bài trước rồi mới trả lời câu hỏi.
Theo kinh nghiệm riêng của mình thì thế này. Với GRE thì nên đọc bài đọc trước và cố gắng hiểu kĩ bài đọc trước khi trả lời câu hỏi. Cách này dùng cho GRE thì tốt vì bài đọc của GRE Reading thường ko quá dài nhưng lại cực khó, câu hỏi thì lắt léo và thiên nhiều về nội dung bài đọc. Do đó, làm bài GRE thường đòi hỏi phải hiểu sâu bài đọc trước thì mới trả lời đúng được đa số câu hỏi.
Còn bài đọc của TOEFL thì tương đối khác. Bài đọc TOEFL thường dài hơn, nên ngay cả khi đọc rất kỹ toàn bài thì đến phần câu hỏi thường là quên và phải đọc lại --> mất thời gian. Hơn nữa, câu hỏi TOEFL ko quá lắt léo và ko thiên quá về nội dung mà bao gồm cả cách bố cục của bài đọc, function của những từ, cụm từ trong bài ... Nhiều câu hỏi lại được đặt song song với phần cần đọc để trả lời Với đặc thù bài đọc và câu hỏi như vậy, ko nhất thiết phải hiểu thật sâu toàn bộ bài mà vẫn có thể trả lời được. Thậm chí có phần ko cần đọc qua vì ko hỏi đến.
Do đó, mình thấy cách làm bài đọc TOEFL tốt nhất đối với mình là đọc lướt qua toàn bộ bài đọc (<1min) xem nó nói về cái gì. Sau đó click ngay sang phần câu hỏi (câu hỏi đặt song song với bài đọc). Đối với những câu hỏi về vocabulary-in-context, referents, hightlighted sentences, sentence inserts, thì đọc từng câu hỏi một và từng đoạn liên quan để đối chiếu và trả lời ngay. Còn những câu hỏi liên quan đến nội dung như details, main ideas, summarizing, inferences .. thì nếu trả lời được luôn qua những đoạn đã đọc thì trả lời luôn. Còn ko thì bỏ qua rồi sau này quay lại đọc thêm những phần cần đọc để trả lời. Câu hỏi summarizing nên làm sau cùng sau khi đã trả lời toàn bộ các câu trước về details. Lúc này nên đọc lại một lượt toàn bài để lọc ra được ý chính.
Theo mình, mỗi người nên practice theo từng cách rồi tự tìm ra cách nào là phù hợp nhất với mình. Khi practice, các bạn sẽ rút ra được kinh nghiệm làm bài riêng rất hữu ích. Khi đó, nhớ share cho những người khác với nhé!
10) Speaking : chỉ nói trong 45s/1 phút thì khó mà dùng những từ cao siêu được. Nên dùng những từ thông thường, nói rõ ràng, trôi chảy, phát triển ý tốt là được rồi.
Về phản xạ bật, nên luyện "thinking and understanding in English without translating". Ngoài ra, nên practice speaking English on daily basis. Cần luyện nói nhiều hằng ngày. Mỗi ngày lấy ra 3, 4 topics (lấy trong file Speaking Notes) để practice. Và practice speaking như trong bộ đề thi thử trên máy nữa. Khi nói thì ko viết ra, mà tập phản xạ y như thi thật luôn. Thậm chí có người còn tập nói mà ko chuẩn bị trước, để rèn phản xạ
Đọc/nghe nhiều cũng giúp bạn vừa có thêm background, vừa học thêm được từ vựng và cách diễn đạt của người bản xứ. Còn đối với phần Integrated thì nên đọc/nghe nhiều, rồi tập tóm tắt và phát biểu cảm nhận, quan điểm riêng về vấn đề mình đọc/nghe.
Còn template thì trong Speaking Notes và forum của thầy Jason đã có rồi. Ngoài ra bạn có thể tham khảo hoặc tự rút ra template riêng của mình từ các sách Cracking, Barron's, Official Guide (ETS). Đó là những sách rất hay.
Cuối cùng là pronunciation. Mình nghĩ accent vẫn có vai trò khá quan trọng trong thi TOEFL. Trên này cũng có 1 bạn nói là người chấm điểm comment cả pronunciation của bạn ý. Chứng tỏ pronunciation tốt vẫn giúp bạn tăng điểm. Ngoài ra, với accent tốt, bạn sẽ thêm tự tin hơn, bài nói cũng sẽ trôi chảy và nghe tự nhiên hơn. Đó cũng là cách ghi điểm tốt.
Về luyện accent và pronunciation theo kiểu Anh Mỹ, mình recommend 2 chương trình cực hay sau đây: "American Accent Training" và "Pronounce It Perfectly in American English" (Barron's). Bạn có thể search và down trên mạng. Luyện theo 2 chương trình này thì sẽ quen với American accent, đặc biệt là cách nối âm (liaison) rất hay gặp trong khi nghe.
11) Listening: Nghe trong practice test đi đã, nghe ít nhất là 2 lần. Lần 1: nghe và trả lời câu hỏi, lần 2: nghe, check đáp án và check cả Transcript. Nếu người nào có thời gian thì có thể nói theo (và check transcript ở những chỗ mình ko nghe thấy). Đó là cách học nghe cẩn thận và lên rất nhanh.
Khi mới bắt bắt đầu thì có thể nghe VOA Special English. Tuy nhiên về lâu dài thì ko nên vì nó quá chậm so với thi thật. Nên nghe VOA News dạng thường, ko phải special (cũng có transcript). Ngoài ra nên nghe thêm CNN, Discovery, NPR, HBO rồi watch movies ... Tập dần với tốc độ nghe nhanh đi để đến hôm thi ko bị choáng
12) Writing
Writing thì thú thực là mình mới chỉ ngồi viết 1 bài integrated trong Kaplan theo đúng thời gian thôi, còn lại chẳng ôn gì nữa cả. Hồi thi GRE mình cũng ôn Writing đúng 1 ngày. Vì mình thấy ôn Writing rất khó. Mình nghĩ cách tốt nhất để viết tốt ko phải là cramming for the exam, mà là cả 1 quá trình, chịu đó đọc nhiều sách báo, write down những từ, cụm từ hay và lạ, viết email bằng tiếng Anh, chat chit bằng tiếng Anh, luyện cách tư duy bằng tiếng Anh ... Tất nhiên là nếu chăm chỉ luyện viết cả TOEFL essays nữa thì chắc điểm sẽ càng tốt hơn.
TÓM LẠI:
Nên luyện càng nhiều sách càng tốt. Song song với đó là làm đề. Làm 7 đề Barron's, 4 đề Kaplan, nếu thừa thời gian thì làm tiếp 6 đề của Cambridge. Tất cả nên làm trên máy.
Làm đề là quan trọng, nhưng khâu quan trọng hơn nữa là check đáp án và đọc explanation & mẫu của họ để xem tại sao mình sai, rút ra kinh nghiệm cho lần sau.
Bên cạnh đó thì viết ra các từ mới mình gặp phải trong lúc nghe/ đọc để học.
Điểm quan trọng cần nhớ là “PRACTICE MAKES PERFECT”. You need to practice a lot. Stick to your plan. Study hard and consistently.
